Property Insurance Claims

Property Damage

  • Protect your property and belongings from further damage (take the necessary steps to minimize the damage).
  • Make reasonable and necessary repairs to protect the property
  • To help file you insurance claim, have an inventory of your possession
  • Notify Insurance Company to report the Claim



Documents Needed


General Requirements

  • Policy/Endorsement
  • Non-Waiver Agreement (Form to be provided by the Adjuster)
  • Sworn Statement of Claim (Form to be provided by the Adjuster)
  • Affidavit of Loss
  • Pictures


Building

  • Proof of Ownership

               - Declaration of Real Property
               - Transfer Certificate of Title
               - Tax Declaration
               - Lease Agreement (if not owner of the building/land)

  • Building Plan/Sketch
  • Repair/Reconstruction Estimate


Contents

  • Proof of Occupancy

                - Declaration of Real Property
                - Transfer Certificate of Title
                - Tax Declaration
                - Lease Agreement (if not owner of the building/land)
                - Mayor&aposs Permit
                - SEC Registration
                - DTI Registration

  • Building Plan/Sketch
  • Inventory of Affected & Unaffected Items showing the acquisition date and cost

  • Repair Estimate, if repairable

Call FGIC Claims Hotline immediately or the branch office servicing nearest you to report the incident.  In case either is not available, you can report a claim online.
